Getting started

You are now ready to begin applying for your AxisPlus benefits. We just wanted to take a moment and explain a few things before you jump in.

1. We have placed a Useful Tips area in the side bar, under the contact info, for portions of this form that may need a little explanation. So, if you do have questions about a specific section or field, you can reference the Useful Tips area.

2. If at any time you aren’t sure and need some clarification throughout this process, don’t hesitate to contact us through chat, phone or email. You can find our contact information in the upper right-hand corner of every page.

3. You should receive an enrollment confirmation email after enrolling, if you do not receive it please check your spam email folder.
